This program is designed to offer extra peace of mind while a homeowner is out of town, as well as provide the department some contact information in the event something happens while the homeowner is out of town. The form can be downloaded and emailed, brought by in person, or mailed to the department. Please notify the department when you return home, so that unfortunate mistakes of identity and actions don't occur.
